He is survived by his father, Gerald Perkisn of Steedman Missouri, his mother Janice Baynes of Moscow Mills Missouri, his son Matthew Cunningham of Cuba Missouri, his brothers, Jason Perkins, Jeffrey Perkins, and Joseph Perkins all of St. Charles Missouri, and his sisters Leah Perkins and Melissah Perkins of St. Charles Missouri, and his grandparents John O. and Mary Lou Perkins of St. Charles Missouri, Bob and Alma Hawkins of St. Charles Missouri, Phyllis Wilson of Tulsa Oklahoma and Tom Wilson of Padre Island Texas
